Dose Range Table
Initial dose: 0.05–0.1 mcg/kg/min. Titrate to hemodynamic response. Central access preferred.
Croup — Nebulized Racemic Epinephrine
Dose: 0.5 mL of 2.25% racemic epinephrine solution
Diluent: Add to 2.5 mL normal saline for nebulization
Route: Nebulizer mask — all pediatric patients regardless of weight
Fixed dose for croup. Monitor for rebound (symptoms may return 2–3h post-treatment). Observe minimum 2–4h after administration. If racemic epi unavailable: L-epinephrine 5 mL of 1:1000 (5 mg) via nebulizer.
