The ANC measures the absolute number of neutrophils in the blood and is a critical marker of infection susceptibility, particularly in oncology patients receiving chemotherapy. Values below 500 cells/µL define severe neutropenia with high febrile neutropenia risk.

ANC = WBC × (Neutrophils% / 100) × 1000
or
ANC = WBC × [(Segs% + Bands%) / 100] × 1000
Clinical Notes
Febrile neutropenia definition: ANC <500 cells/µL + fever ≥38.3°C (single) or ≥38.0°C sustained >1 hour.

Chemotherapy nadir: Typically 7–14 days post-treatment; recovery by 21–28 days in most regimens.

MASCC score is used for febrile neutropenia risk stratification to determine outpatient vs. inpatient management eligibility.

G-CSF (filgrastim/pegfilgrastim) use for prophylaxis is indicated when febrile neutropenia risk exceeds 20% based on regimen and patient factors.
