2020 ASHP/IDSA Guidelines: AUC/MIC monitoring is now preferred over trough-only monitoring. Target AUC 400–600 mg·h/L assuming MIC ≤1 mg/L. Empiric starting dose typically 15–20 mg/kg Q8-12h. Obtain 2 vancomycin levels at steady state for Bayesian AUC estimation. This calculator provides initial empiric dosing estimate only.
