TIMI Risk Score — NSTEMI/UA
14-day risk of death, MI, or urgent revascularization in NSTEMI/unstable angina
Overview
The TIMI Risk Score for NSTEMI/UA uses 7 binary variables to predict 14-day risk of all-cause mortality, new or recurrent MI, or need for urgent revascularization.

14-Day Event Rate by Score
| Score | 14-Day Event Rate | Risk Category |
|---|---|---|
| 0–1 | ~3% | Low |
| 2 | ~5% | Low |
| 3 | ~13% | Intermediate |
| 4 | ~19% | Intermediate |
| 5 | ~26% | High |
| 6–7 | ~41% | High |
Score = sum of 7 binary variables (0 or 1 each)
Max = 7 | Low: 0-2 | Intermediate: 3-4 | High: 5-7
Max = 7 | Low: 0-2 | Intermediate: 3-4 | High: 5-7
Clinical Notes:
• High scores benefit from early invasive strategy and aggressive antiplatelet therapy
• Aspirin use as risk factor reflects treatment failure / severe disease
• Original TIMI NSTEMI trial validated in ACS patients
• Consider GRACE score for complementary risk stratification
