Overview
Mean PAP (mPAP) is the key diagnostic criterion for pulmonary hypertension. Per 2022 ESC/ERS guidelines, mPAP >20 mmHg defines pulmonary hypertension. Calculate from echo or right heart cath data.

Clinical Notes:
• Echo estimation of PAP has significant limitations — right heart catheterization remains the gold standard
• PH confirmed only by RHC: mPAP >20 mmHg at rest
• Pre-capillary PH (Group 1, 3, 4, 5): PCWP ≤15 + PVR ≥2 WU
• Post-capillary PH (Group 2): PCWP >15 mmHg
