MEWS Escalation Thresholds
ScoreRiskAction
0–4LowRoutine monitoring; increase frequency if trending up
5–6MediumNotify nurse in charge; increase monitoring frequency; consider medical review
≥7HighNotify doctor immediately; consider MET/RRT activation; urgent assessment
MEWS = SBP score + HR score + RR score + Temp score + AVPU score
Range: 0–14
Clinical Note: MEWS is most useful for trend monitoring — a rising score warrants escalation even if absolute value is low. Different institutions may use NEWS (National Early Warning Score) with slightly different thresholds. Single elevated parameter ≥3 should trigger review regardless of total score.
