MEWS Escalation Thresholds

| 0–4 | Low | Routine monitoring; increase frequency if trending up |
| 5–6 | Medium | Notify nurse in charge; increase monitoring frequency; consider medical review |
| ≥7 | High | Notify doctor immediately; consider MET/RRT activation; urgent assessment |

Clinical Note: MEWS is most useful for trend monitoring — a rising score warrants escalation even if absolute value is low. Different institutions may use NEWS (National Early Warning Score) with slightly different thresholds. Single elevated parameter ≥3 should trigger review regardless of total score.
