| Lithium Level (mEq/L) | Clinical Status | Signs / Action |
|---|---|---|
| 0.6–0.8 | Maintenance therapeutic | Standard maintenance range |
| 0.8–1.2 | Acute mania therapeutic | Standard acute treatment range |
| 1.2–1.5 | High-normal | Monitor closely; fine tremor, GI upset |
| 1.5–2.0 | Early toxicity | Coarse tremor, ataxia, confusion — reduce dose |
| > 2.0 | Severe toxicity | Seizures, arrhythmia — hold Li; supportive care; HD if severe |
Drug Interactions: Increase Li levels: NSAIDs (indomethacin, ibuprofen), thiazide diuretics, ACE inhibitors, ARBs, low-sodium diet. Decrease Li levels: caffeine, theophylline, acetazolamide. Monitor levels weekly initially, then every 3–6 months. Draw level 12 hours after last dose (trough). Ensure adequate hydration and sodium intake. Monitor renal function and thyroid q6–12 months.
