IBW male = 50 + 2.3 × (Height_inches − 60)
IBW female = 45.5 + 2.3 × (Height_inches − 60)
ABW = IBW + 0.4 × (ActualWt − IBW) [use if obese]

Clinical Notes: Use actual body weight unless patient is obese (BMI >30), then use adjusted body weight (ABW). If actual weight < IBW, use actual weight. Cockcroft-Gault (not eGFR) is used for most drug dosing decisions per FDA and pharmacokinetic studies.
